
Admission Process
Evergreen Estates is required by Washington state to complete a pre-admission assessment for our Assisted Living residents. This assessment is completed by our Director of Nursing.
The following is the step-by-step admission process:
Step 1 - The Deposit
Evergreen Estates Assisted Living Community requires a $400 deposit. This ensures the room you desire is reserved during the admission process. If it is decided during admission that you will not be admitted to Evergreen Estates, your deposit will be refunded in full. If you are admitted, the deposit becomes non-refundable and is not considered part of your monthly rent.
Step 2 - The Nursing Assessment
Our Director of Nursing will complete this process, usually face-to-face. The assessment is designed to judge your abilities to live in our community safely. You will be asked to demonstrate your physical abilities and asked some personal questions. The process can take up to 2 hours. You will be told the results of your assessment within 1 business day.We ask that you sign a ROI in order to gather information from your doctor.
Step 3 - The Gathering
This is where you need to find insurance cards, Social Security cards, Guardianship papers, Power of Attorneys, phone numbers and addresses of relatives. You will need to bring all of this information with you on your move-in day.
Step 4 - The Paperwork
In this face-to-face meeting, we will begin gathering information from appropriate local healthcare facilities. This may include requesting records from physicians, hospitals and others. In order to request this information, you will need to sign a document that says we are allowed access. Unfortunately, if you choose not to sign the document, your admission will be declined.
This process can take a long period of time, causing frustration. You will be kept abreast of our progress and you may be asked to assist us in gathering the needed paperwork to speed up the process.
Step 5 - The Physician
Another requirement in the admitting process is that each resident be under the care of a licensed physician in the area. If you or your loved one do not have a physician in the area, you will need to obtain one prior to being admitted to Evergreen Estates. Physicians will often require an office visit from new patients. We would be happy to assist you in your search, but as this is a sensitve and personal choice, we do not make recommendations.
Step 6 - Admitting Orders
Evergreen is required by law to have signed admitting orders from your local physician. We ask that you allow us to initiate this request, and do so often in Step 1 of the process.
Step 7 - Move-In Day
Once you or your loved one has been approved for admission, with all the necessary paperwork, we will arrange a move-in day with you. On this day, someone from the front office will go through the Admission Agreement form with you. This is the time to present us with any legal Guardianship or Power of Attorneys papers. You will also be asked for your first monthly payment.
Step 8 - The Care Plan
The Care Plan process begins after you have moved in. Based on the information gathered about you or your loved one, we start building the Care Plan. This plan describes, in detail, what services our staff will need to provide and what your physical and mental limitations may be. Your input is greatly appreciated.
You and/or your loved one will be asked to participate in the Care Plan review process. During this time the Director of Nursing will sit down with you to review your care needs and any other information we feel is appropriate.
