Care Plan Process

The Care Plan process is started once you or your loved one has moved into Evergreen Estates.

We start gathering information about you or your loved one at the beginning of the admission process. Based on this information, we develop the Care Plan, which details which services we will provide and what your physical and mental limitations may be. You will have the opportunity to give us your input about this information.

Once the plan has been prepared, you and/or your loved one will be asked to help review it. Our on-staff nurse will review your care needs and other information with you during this time. Please be aware that the Care Plan is an honest assessment of you or your loved one; hence, you may not always agree with what is in the plan or may not have noticed the things we see.
